Every employee on the Argos medical plan must complete the Tobacco Affidavit, attesting that you are tobacco-free, or complete the Tobacco Cessation Program. Failure to do so will result in the employee paying a $50 monthly surcharge, totaling $600 next year.g $600 next year.
How to Complete:
Head to our Argos Live Well Portal at https://argoslivewell.livehealthyignite.com/users/sign_in
On the Dashboard, click Complete This next to Tobacco Affidavit under Required Activities.
Select “I am not a tobacco user” and submit.
OR
- Go to the Challenges tab and then click the Video Courses tab.
- Click the Join button on the Tobacco Cessation Course to get started.
- Watch each video and pass the quiz afterward.
Then go back to the Tobacco Affidavit button and attest that “I am a tobacco user and attest that I have completed the Tobacco Cessation Video Learning Course.”
